Description:

THIS JOB IS UNDER REVIEW BY TOTAL COMPENSATION AND AS A RESULT THE FLSA STATUS MAY CHANGE. Provides high quality and cost effective project management, consulting, and analytic support. Assists senior consultants, clients, and/or groups in identifying and resolving issues of strategic importance to the organization. May assist senior consultants or manage complex projects independently.

Essential Responsibilities:

  • Defines and frames complex issues and develops time tables/processes for decision making.
  • Translates needs, issues, and ideas into effective strategies and action plans.
  • Coordinates team/project activities and schedules.
  • Researches key business issues, and directs the collecting and analyzing of quantitative and qualitative data.
  • Synthesizes analysis and identifies key findings.
  • Assists in the determination of goals and priorities with team, clients, or project management sponsors.
  • Establishes team membership and project participants.
  • Develops proposals for clients outlining proposed project structure, approach and work plan.
  • Provides staff leadership to project teams, as well as manages work of outside consultants when needed.
  • Formulates specific implementation plans and evaluates the effectiveness of actions/programs implemented.
  • Communicates results/recommendations to project sponsors.
  • Develops creative alternative solutions.
  • Recommends action steps to team and client.
  • Plans and facilitates meetings.
  • Makes formal presentations to various senior level audiences.
  • Transfers project team knowledge/tools to client.
  • Participates in the development and management of the department, including coaching, recruiting, conducting performance reviews for analysts, and other departmental activities.

    Professional liability indemnification is provided to all clinicians in the Medical Group. Kaiser Permanente’s Indemnification Program has a self funded base in addition to an excess insurance component. The Indemnification Program indemnifies each clinician for all professional acts within the course and scope of their employment throughout dates of employment in the Medical Group.

    Comprehensive Kaiser Permanente medical coverage is provided for Associate Medical Group members, Shareholders, and eligible dependents. Employees pay a subsidized, pre-tax share of the monthly premium, which depends on the family coverage desired, and pay the full premium cost attributable to non-registered domestic partners (“DP”).

    To receive an extra $20 per month premium offset, the employee can complete four criteria available from the benefits department. To receive an additional $20 per month premium offset, an employee’s spouse or registered domestic partner can complete three criteria.

    Comprehensive dental insurance coverage is provided for Associate Medical Group members, Shareholders, and eligible dependents.

    Associate Medical Group members and Shareholders receive fully vested, employer-paid contributions to WPMG’s 401(k) Retirement Savings Plan. These flat-rate, non-discretionary contributions are made by WPMG every pay period, according to a specific formula.

    Individuals must first complete one year of service (with 1,000 or more hours worked) before entering the plan on the next quarter entry date (first of January, April, July, or October). Participants are vested immediately at 100%. On top of that, WPMG may make an additional, discretionary profit sharing contribution each year. The Plan offers a choice of investment options.

    WPMG offers Medical Group members, regardless of FTE, an opportunity to make voluntary pre-tax or Roth after-tax contributions to our 401(k) Retirement Savings. Effective January 1, 2018, the maximum allowable contribution is $18,500. Individuals 50 or older during any part of 2018 may also make an additional $6,000 catch up contribution, for a total of $24,500. The Plan offers a choice of investment options.

    A supplemental retirement savings plan may also be available. The cash balance plan provides an additional opportunity to save on a pre-tax basis. Participation levels are pre-defined. Detailed communications will be sent to new staff prior to their start date. Eligible individuals will be required to make an irrevocable election regarding participation in the plan before starting to work at WPMG.

    A $250,000 (benefit pro-rated based on FTE) basic term life insurance policy with a $250,000 Accidental Death & Dismemberment (AD&D) component is offered for each member, with decreasing coverage limits after age 70. The life insurance increases from $250,000 to $300,000 (prorated by FTE) upon reaching Shareholder status. You may purchase supplemental life insurance or supplemental AD&D insurance for up to five times base pay, not to exceed $1,500,000. Spousal coverage may also be purchased.

    Each member of the Medical Group accrues four weeks of paid vacation (benefit pro-rated based on FTE and adjusted to the individual work schedule). In addition, WPMG recognizes eight holidays per year. Holiday time is prorated based on FTE. Vacation for certain departments is built into their schedules.

    Each member will accrue 1 day per month of sick leave for each month worked, up to a maximum of 6 weeks.

    Salary while on short-term disability is paid at 80%. Sick leave and short-term disability total approximately 13 weeks of pay in a rolling 12-month period. Short-term disability begins when sick leave is exhausted if an employee is out for their own illness or injury.

    Long-term disability coverage begins after 90 days of sick leave and/or short-term disability, as long as a claim has been filed and approved. The LTD benefit allows up to 55% of monthly income for members disabled beyond 90 days. Individuals may purchase supplemental long-term disability coverage to increase to 60% income replacement.

    A basic long-term care insurance plan is provided. Long-term care insurance offers financial assistance for care through a nursing home facility, professional home care, or an assisted living facility. Individuals may purchase higher levels of coverage to expand their basic plan, at competitive group rates.

    During the Associate Staff period, full-time Medical Group members on a standard 5-day work week are provided seven days for the purpose of postgraduate education with $2,600 in funding for education and professional society dues (prorated based on FTE). Upon reaching Shareholder status, full-time Medical Group members on a standard 5-day work week are provided ten days for the purpose of postgraduate education with $3,800 in funding for education and professional society dues (prorated based on FTE).

    After the first day of employment, Associate Medical Group members may apply to their respective Benefit Coordinator for reimbursement of the full cost of professional license renewal fees (Washington State and DEA renewals) and for expenses related to initial board examination or recertification examinations. Expenses incurred prior to joining WPMG will not be reimbursed. Reimbursements are part of the postgraduate expense allowances referenced above.

    New Associate staff members moving 100 miles or more are eligible for the standard moving benefit of 100% of the first $7,000 in approved moving expenses.

    Shareholders may apply for a sabbatical after 10 years of employment or 10 years after commencement of their last sabbatical. Sabbaticals will be WPMG funded at the following percentage of base salary: Months 1-3 at 20%, Months 4-6 at 15%, Months 7-9 at 10%, Months 10-12 at 5%.

    Additional benefits include Flexible Spending Accounts, ID Theft Protection, and Emergency Travel Assistance. Please contact the Benefits Team for additional information.

    Benefits as described are currently available to Associate Medical Group Members and Shareholders. Details of each benefit are available from the WPMG Benefits Department. Most benefits become effective on the first day of the month following appointment. Benefits are provided to associates and shareholders working at least a 0.50 FTE unless otherwise noted. The WPMG Board reserves the right to change or terminate WPMG benefit programs. This is only a summary; the language of the Plan Documents and applicable laws will control.

    Why I Left Kaiser (and Why You Might Want To)

    I made a career change most ophthalmologists don’t even consider:  I resigned from Kaiser voluntarily after making partner/shareholder.  In an era where solo practitioners are trying to join a bigger organization and new graduates drool over a Kaiser offer, I have found that the “Kaiser way” may not be the “right way.”  

    I first joined the Permanente Medical Group fresh out of training and thought how lucky I was to have landed such a coveted position.  Conventional beliefs have it that the Kaiser salary (plus benefits) was significantly higher than what one would be make in private practice or in academia.  Also, I wouldn’t have to work unethically to sell premium lenses or knock on doors for patient referrals.  

    While some of these advantages (such as patient referrals) still hold true and are the reasons for Kaiser’s attractiveness to some candidates, let me share some Kaiser “secrets” to provide a clearer depiction of the organization so that one can make an informed career decision.   

    For those who are not familiar with the Kaiser system, here is a very abbreviated explanation.  Kaiser Permanente is an HMO, and consists of the Kaiser Foundation Health Plan (the Kaiser insurance) and the Permanente Medical Groups (the Kaiser doctors).  The patients buy the Kaiser insurance and they are only allowed to see Kaiser doctors.  The Kaiser doctors are salaried from the pool of insurance premiums and provide service only to the Kaiser patients.  In the Kaiser model, services are prepaid, so there is no financial incentive or penalty to providing or receiving medical care.  

    The first myth I’d like to debunk is “Kaiser’s starting salary is significantly higher than one would make in private practice.”  The way Kaiser sets its salary is that we take the Medscape survey median and add a preset boost.  So for ophthalmology, the Medscape median is 180K, then depending on your region, an additional +/-100K gives an entry-level salary of ~280K for a 1.0 FTE (full-time employee).  This seems substantial to a debt-ridden resident, but one may not understand two key flaws in this number.  First of all, the Medscape salary survey is not accurate:  it is self-reported and it lumps everyone together, so that 180K is very low for a full-time ophthalmologist.  Second, private practice is performance-based, so the more patients you see, the more you will make.  Kaiser makes everyone see a certain number of patients with 30-40% new consults and 60-70% follow-ups.  A Kaiser ophthalmologist could make around 500K in private practice if s/he sees the same number of patients with the same new-to-follow-up ratio.  So Kaiser is actually paying their ophthalmologists 40% less than what they should make.  And Kaiser does not allow for negotiation of compensation. 

    The biggest gripe I’ve heard as a chief and experienced as a Kaiser physician is the lack of flexibility or the lack of autonomy.  Clinic, OR, and call schedules are made 6-months in advance.  The clinic template is also preset.  If you want to start clinic a little bit later, that’s not possible.  If you want to leave clinic a little bit earlier, that’s not possible.  If you don’t want to take an hour for lunch, that’s not possible.  If you don’t want to work a full FTE but your center needs you to work a full FTE, that’s not possible.  This rigidity may seem ridiculous but it is what all Kaiser physicians live by.  

    The metrics by which a Kaiser physician is evaluated against are somewhat unintuitive.  This is because all specialties use the same metrics, be it ophthalmology or neurology or neurosurgery.  One such metric is a patient satisfaction score (MPS score).  This number comes from a patient satisfaction survey in which only 5 questions out of a 25-question survey pertains to physician performance.  We all know that surveys are prone to survey bias, but Kaiser uses this MPS score as a physician performance score.  Another guideline is “same-day/next-day access”, meaning that a patient requesting a referral is seen the same day or the next day.  This immediate access may make sense for medical subspecialties or general surgery. but for ophthalmology or optometry, it makes little sense.  Having a cataract evaluation tomorrow is not as important as having a cataract evaluation next Wednesday when you have time off of work to go see the ophthalmologist.  But in this example, the physician gets a negative point for tailoring to the patient's preference versus forcing the patient to take the same-day or next-day slot.  To be an all-star Kaiser physician, one simply needs to meet the “same-day/next-day” access and perform the blood pressure/flu shot/colon cancer screening (yes, all ophthalmologists have to give patients the stool occult kit).

    I often jokingly compare the Kaiser environment to socialism to my non-Kaiser friends.  There is some truth to this.  Although Kaiser does not push its providers to sell premium lenses or perform costly procedures, there is also no incentive to perform.  One can be a mediocre physician and get the same salary as an overachieving physician, so there is less motivation to do more than mediocre.  Just as one cannot negotiate the salary, incentive pay and annual increases are also predetermined with no room for negotiation.  Lastly, remember that in the Kaiser model, services are prepaid, so there is no financial incentive to provide medial care.  In fact, there is incentive to providing less medical care, because the less care Kaiser gives, the more money Kaiser keeps.  While each visit in private practice generates revenue, each non-visit at Kaiser generates revenue.  So for a condition such as POAG that may generate 3-4 visits annually to the ophthalmologist in private practice, at Kaiser, it is compressed into 1-2 visits.  

    There are many physicians who fit the Kaiser model and are happy working for Kaiser. For the newly minted residents and fellows who are wondering if the “Kaiser way” is “the way,” understand that the “Kaiser way” is not “the right way” or “the only way.”  When you are considering your options, don’t be too envious of your Kaiser friends, and don’t assume private practice is “the wrong way."  You have to find what’s the best fit for your personality and your lifestyle and desires.  This article is simply here to provide you with additional information about Kaiser to help you make that decision.
