What is the most important information I should know about ABSORICA?

ABSORICA can harm your unborn baby, including birth defects (deformed babies), loss of a baby before birth (miscarriage), death of the baby, and early (premature) births. Patients who are pregnant or who plan to become pregnant must not take ABSORICA.
Patients must not get pregnant:

  • for 1 month before starting ABSORICA
  • during treatment with ABSORICA
  • for 1 month after stopping ABSORICA

If you get pregnant during treatment with ABSORICA, stop taking it right away and call your healthcare provider.

Patients taking ABSORICA must register in the iPLEDGE® Pregnancy Registry at or www.ipledgeprogram.com. See your healthcare provider for further information.

ABSORICA can cause serious mental health problems, including:

  • depression, psychosis (seeing or hearing things that are not real)
  • suicide. Some patients taking ABSORICA have had thoughts about hurting themselves or suicide. Consult your healthcare provider if you have such thoughts.

Stop taking ABSORICA and call your healthcare provider right away if you or a family member notices any signs or symptoms of depression or psychosis.

What are the possible side effects of ABSORICA?

ABSORICA can cause serious side effects, including:

  • increased pressure in the brain (intracranial hypertension). ABSORICA can increase the pressure in your brain. This can lead to permanent loss of eyesight, and in rare cases, death. Stop taking ABSORICA and call your healthcare provider right away if you get any of these signs of increased brain pressure:
    • bad headache
    • blurred vision
    • dizziness
    • nausea or vomiting
    • seizures (convulsions)
    • stroke
  • serious skin problems. Skin rash can occur in patients taking ABSORICA. Sometimes rash can be serious and may lead to death. Stop using ABSORICA and call your healthcare provider right away if you get:
    • conjunctivitis (red or inflamed eyes, like “pink eye”)
    • rash with a fever
    • blisters on legs, arms or face
    • sores in your mouth, throat, nose or eyes
    • peeling of your skin
  • inflammation of your pancreas (pancreatitis) can happen in patients who take ABSORICA and can lead to death. Call your healthcare provider right away if you have any of the following symptoms of pancreatitis:
    • severe upper stomach (abdomen) pain
    • swelling of your stomach
    • nausea and vomiting
    • fever
  • increased blood fat (lipid) levels. ABSORICA can raise blood fat levels (cholesterol and triglycerides). Your healthcare provider will do blood tests to check your lipids before and during treatment. These problems usually go away when ABSORICA treatment is finished.
  • hearing problems. Stop using ABSORICA and call your healthcare provider if your hearing gets worse or if you have ringing in your ears. Your hearing loss may be permanent.
  • liver problems, including hepatitis. Your healthcare provider will do tests to check your liver before and during treatment with ABSORICA.
  • inflammation of your digestive tract (inflammatory bowel disease). Stop taking ABSORICA and call your healthcare provider.
  • bone and muscle problems. Bone problems include bone pain, softening or thinning (which may lead to fractures). Tell your healthcare provider if you plan hard physical activity during treatment with ABSORICA.

Stop ABSORICA and call your healthcare provider right away if you have muscle weakness, which can be a sign of serious muscle damage.

ABSORICA may stop long bone growth in teenagers who are still growing.


  • vision problems. Stop taking ABSORICA and call your healthcare provider right away if you have any vision changes. ABSORICA may affect your ability to see in the dark. Some patients get dry eyes during treatment. If you wear contact lenses, you may have trouble wearing them during and after you stop treatment with ABSORICA.
  • serious allergic reactions. Stop taking ABSORICA and get emergency medical help right away if you get hives, a swollen face or mouth, or have trouble breathing. Stop taking ABSORICA and call your healthcare provider if you get a fever, rash, or red patches or bruises on your legs.
  • blood sugar problems, including diabetes. Tell your healthcare provider if you are very thirsty or urinate more than usual.

1. About isotretinoin capsules

Isotretinoin capsules are a very effective treatment for severe acne (spots).

This medicine can have serious side effects, however, so it must be prescribed and supervised by a specialist doctor.

Isotretinoin capsules are also known by the brand names Roaccutane and Rizuderm.

2. Key facts

  • Isotretinoin capsules start to work after a week to 10 days. They work very well - 4 out of 5 people who use them have clear skin after 4 months.
  • You will a have blood test before you start taking isotretinoin and regular blood tests during your treatment.
  • Your skin may become very dry and sensitive to sunlight during treatment. Using lip balm and moisturisers will help.
  • It's very important not to become pregnant while using isotretinoin capsules and for at least 1 month after stopping. This is because isotretinoin can harm an unborn baby. Women will be given a reminder card about this.
  • If you become depressed or think about harming yourself while taking isotretinoin, stop taking the medicine and tell your doctor straight away.

3. Who can and can't take isotretinoin capsules

Isotretinoin capsules are for teenagers and adults with severe acne. Do not give isotretinoin capsules to children under the age of 12 years or before puberty.

Isotretinoin capsules aren't suitable for some people.

Do not take isotretinoin capsules if you:

  • have had an allergic reaction to isotretinoin, soya (the capsules contain soya) or any other medicines in the past
  • have an inherited digestive disorder called fructose intolerance (the capsules contain sorbitol)

To make sure isotretinoin capsules are safe for you, tell your doctor if you:

  • have had a mental health illness like depression
  • are pregnant or think you could be, or you're breastfeeding
  • have ever had an allergic reaction to isotretinoin or any other medicine
  • have liver or kidney disease
  • have high levels of cholesterol or other fats in your blood
  • have high levels of vitamin A
  • have Crohn's disease or ulcerative colitis

If you have diabetes, talk to your doctor before beginning treatment with isotretinoin capsules. You may need extra monitoring while you take isotretinoin capsules as this medicine can cause a rise in blood sugar levels.

6. How and when to take it

It's important to take the capsules as instructed by your doctor.

As a general rule, the starting dose is mg per kilogram body weight per day ( mg/kg/day). So if you weigh 60kg, your dose will usually start at 30mg a day.

Usually, you take isotretinoin capsules once or twice a day. Each capsule contains a dose of 10mg or 20mg of isotretinoin.

The dose of isotretinoin capsules varies from person to person, and depends on your weight.

Swallow the capsules whole with a drink of water. It's best to take them straight after a meal or snack to make sure they work properly.

What if I forget to take it?

If you take the capsules once a day and remember you've missed a dose on that day, take it as soon as you remember. If it's the next day when you remember, skip the missed dose and take the next capsule at the usual time.

If you take the capsules twice a day and forget to take a dose, take it as soon as you remember, unless this is within 2 hours of your next dose. In this case, skip the missed dose and take the next capsule at the usual time.

Never take a double dose. Never take an extra dose to make up for a missed one.

If you often forget doses, it may help to set an alarm to remind you. You could also ask your pharmacist for advice on other ways to help you remember to take your medicine.

What if I take too much?

If you need to go to your nearest A&E department, take the isotretinoin packet or leaflet inside it plus any remaining medicine with you.

Will the dose I take go up or down?

After a few weeks, your doctor may change your dose of capsules. You may need a higher dose if your acne isn't getting better. You may need a lower dose if you have side effects that bother you.

7. Advice for women

This medicine is likely to harm a baby. It also increases the risk of miscarriage.

If you become pregnant during treatment with isotretinoin capsules, stop taking the capsules and tell your doctor as soon as possible.

It's very important that you do not get pregnant while you're taking isotretinoin capsules. You'll be asked by your doctor to follow strict rules to prevent pregnancy during treatment and for 1 month afterwards.

Before starting treatment with isotretinoin capsules, women who are able to become pregnant must agree to:

  • use at least 1, and ideally 2, reliable methods of contraception for 1 month before starting isotretinoin capsules, and for 1 month after treatment has stopped - the second contraceptive should be a barrier method of contraception (for example, a condom), but you shouldn't use barrier methods on their own
  • have a pregnancy test before, during, and 5 weeks after the end of treatment - some doctors may ask you to have monthly pregnancy tests

Your doctor will give you a patient reminder card for isotretinoin. Read this card and the information leaflet that comes with your medicine before you start taking the capsules.

Do not breastfeed while you're using isotretinoin capsules. This medicine can get into breast milk and harm your baby.

Important

Do not take isotretinoin capsules if you think you're pregnant, you know you're pregnant, or if you're breastfeeding.

Men and isotretinoin capsules

Men can safely take isotretinoin capsules if they and their partner are trying for a baby, or their partner is pregnant.

Isotretinoin capsules don't seem to damage sperm. Only tiny amounts of isotretinoin get into semen, which is too little to harm the unborn baby.

8. Cautions with other medicines

Some medicines and isotretinoin capsules can interfere with each other, and make it more likely that you'll have side effects.

Tell your doctor if you're taking these medicines before you start treatment with isotretinoin capsules:

  • supplements that contain vitamin A
  • tetracycline antibiotics such as doxycycline, oxytetracycline, minocycline, and lymecycline
  • other acne medicines (using both together may make skin irritation worse)

Mixing isotretinoin capsules with herbal remedies or supplements

There's very little information about taking herbal remedies and supplements while you use isotretinoin capsules.

Tell your doctor or pharmacist if you're taking any other medicines, including herbal remedies, vitamins or supplements.

You must have a negative pregnancy test before you start taking isotretinoin, before each prescription is refilled, right after you take your last dose of isotretinoin, and again 30 days later. All pregnancy testing is required by the iPLEDGE program.

You must agree in writing to use two specific forms of birth control beginning 30 days before you start taking isotretinoin and ending 30 days after your last dose. Both a primary and a secondary form of birth control must be used together.

Primary forms of birth control include:

  • tubal ligation (tubes tied);
  • vasectomy of the male sexual partner;
  • an IUD (intrauterine device);
  • estrogen-containing birth control pills (not mini-pills); and
  • hormonal birth control patches, implants, injections, or vaginal ring.

Secondary forms of birth control include:

  • a male latex condom with or without spermicide;
  • a diaphragm plus a spermicide;
  • a cervical cap plus a spermicide; and
  • a vaginal sponge containing a spermicide.

Stop using isotretinoin and call your doctor at once if you have unprotected sex, if you quit using birth control, if your period is late, or if you think you might be pregnant. If you get pregnant while taking isotretinoin, call the iPLEDGE pregnancy registry at

Not having sexual intercourse (abstinence) is the most effective method of preventing pregnancy.

You should not use isotretinoin if you are allergic to it.

Tell your doctor if you have ever had:

  • depression or mental illness;
  • asthma;
  • liver disease;
  • diabetes;
  • heart disease or high cholesterol;
  • osteoporosis or low bone mineral density;
  • an eating disorder such as anorexia;
  • a food or drug allergy; or
  • an intestinal disorder such as inflammatory bowel disease or ulcerative colitis.

You should not breastfeed while using this medicine.

Isotretinoin is not approved for use by anyone younger than 12 years old.

Follow all directions on your prescription label and read all medication guides or instruction sheets. Use the medicine exactly as directed.

Each prescription of isotretinoin must be filled within 7 days of the date it was written by your doctor. You will receive no more than a day supply of isotretinoin at one time.

Always take isotretinoin with a full glass of water. Do not chew or suck on the capsule. Swallow it whole.

Follow all directions about taking isotretinoin with or without food.

Use this medicine for the full prescribed length of time. Your acne may seem to get worse at first, but should then begin to improve.

You may need frequent blood tests.

Never share this medicine with another person, even if they have the same symptoms you have.

Store at room temperature away from moisture, heat, and light.

Skip the missed dose and use your next dose at the regular time. Do not use two doses at one time.

Seek emergency medical attention or call the Poison Help line at Overdose symptoms may include headache, dizziness, vomiting, stomach pain, warmth or tingling in your face, swollen or cracked lips, and loss of balance or coordination.

Do not take a vitamin or mineral supplement that contains vitamin A.

Do not donate blood while taking isotretinoin and for at least 30 days after you stop taking it. Donated blood that is later given to a pregnant woman could lead to birth defects in her baby if the blood contains any level of isotretinoin.

While you are taking isotretinoin and for at least 6 months after your last dose: Do not use wax hair removers or have dermabrasion or laser skin treatments. Scarring may result.

Isotretinoin could make you sunburn more easily. Avoid sunlight or tanning beds. Wear protective clothing and use sunscreen (SPF 30 or higher) when you are outdoors.

Avoid driving or hazardous activity until you know how this medicine will affect you. Isotretinoin may impair your vision, especially at night.

Get emergency medical help if you have signs of an allergic reaction (hives, difficult breathing, swelling in your face or throat) or a severe skin reaction (fever, sore throat, burning eyes, skin pain, red or purple skin rash with blistering and peeling).

Stop using isotretinoin and call your doctor at once if you have:

  • problems with your vision or hearing;
  • muscle or joint pain, bone pain, back pain;
  • increased thirst, increased urination;
  • hallucinations, (see or hearing things that are not real);
  • symptoms of depression--unusual mood changes, crying spells, feelings of low self-worth, loss of interest in things you once enjoyed, new sleep problems, thoughts about hurting yourself;
  • signs of liver or pancreas problems--loss of appetite, upper stomach pain (that may spread to your back), nausea or vomiting, fast heart rate, dark urine, jaundice (yellowing of the skin or eyes);
  • severe stomach problems--severe stomach or chest pain, pain when swallowing, heartburn, diarrhea, rectal bleeding, bloody or tarry stools; or
  • increased pressure inside the skull--severe headaches, ringing in your ears, dizziness, nausea, vision problems, pain behind your eyes.

Common side effects may include:

  • dryness of your skin, lips, eyes, or nose (you may have nosebleeds);
  • vision problems;
  • headache, back pain, joint pain, muscle problems;
  • skin reactions; or
  • cold symptoms such as stuffy nose, sneezing, sore throat.
